OpenAI 429 insufficient_quota / billing_hard_limit_reached

OpenAI returns HTTP 429 with code insufficient_quota when prepaid credits expire or monthly spend cap is hit; retrying against api.openai.com will fail indefinitely until failover.

1. Immediate Code & SKU Fix

Replace With SKU: openrouter (openai/gpt-5 or anthropic/claude-sonnet-4-5)

Install Command: npm install openai@latest

Required Env Vars: OPENROUTER_API_KEY
